For SWAT Registration:
For those NEW TO UYSA, make sure you have an electronic FACE ONLY photo of your player AND electronic copy of your players BIRTH CERTIFICATE accessable on your hard drive. You will be asked to upload both of these during the registration process.
Dont have an electronic copy of the birth certificate? One helpful trick is to take a photo of the birth certificate with a smart phone and email it to yourself. Save the picture to your computer and you have an electronic copy without having to use a scanner.
******RETURNING PLAYERS if you do not have your username and password please use the forgot username / password link. The system will also allow you to create a username and password if you are in the system and have an email on file in your account.
Simply provide your email address (must be on file in the system), last name, and zip code. You will recieve an email asking you to reset password. Choose this option and you will be able to create a username if needed, as well as a password.
If you have multiple accounts please contact Affinity Sports @ 1-888-211-3444 to have your accounts merged.
